Hi,

Could someone please ID this bush to me and also does it look like it is dying ?

I think it's Kerria, a deciduous shrub with yellow flowers. Kerria produces suckers and spreads, forming dense thickets if unchecked. If you look closely at some of the green leaves low down, they also look like the leaves of Kerria. Your plant looks pretty unhappy - it's very congested and there seem to be lesions on many of the upright branches. Congestion encourages fungal attacks. It also looks as if it has been trimmed hedge-fashion to below its normal height, which might be around 8 feet or so. Constant trimming is probably removing the flowering wood too. However some of those shoots with green leaves still look healthy. If you want to keep the shrub I would dig it all out, but replant some of the healthy looking bits. However, if you don't want a plant 8' high, then choose another shrub, maybe a good hedging plant which can be constantly trimmed.
