it might be worth checking for vine weevil in thecompost, everytime i have seen heucheras looking like yours are sounding, they normally are the ones causing the problems, little blitters

they could be ok possibly, its so differicult to say with phytoria, i would stick my neck out and say they should be ok, it is such a ramdom killer

wouldn't bother with deadheading the hebe, no real advantage to be have doing it.
with the scabious i would remove the stem right the way down to the base of the plant, the chilean black form i would remove the spent flowers down to the next stem or bud, ideally once a week if possible

i hate to say it but i think it is dying, the symptons sound like a form of Phytophthora which basically attachs the roots of the plant and can kill it totally within days, once saw a 100ft cedar in prefect health die within a week from it, in the way basically you discribe
