O. monacantha is an upright, irregular, shrub-like, borderline hardy, evergreen perennial cactus with branched stems divided into flattened, fleshy, spiny, broadly ovate, bright-green segments. Red-flushed, yellow flowers in summer are followed by spherical, deep red fruit.
